You can pre-plan you funeral by using the form here - Pre-Planned Funeral Form

The form will be forwarded to the Funeral Home of your choice. If you use this pre-planned form and request a follow-up from the Funeral Home, they will give you a 10% discount toward a conversion to a pre-arranged or pre-paid funeral.
